29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  JOB SITE: 11871 East State Highway E, Cadet, MO 63630, on or about 4/25/2016, the employer failed to provide adequate point of operation guards to prevent employee from having any part of their body in the danger zone during operating cycle:   1) A Morgan 14" Chop Saw with the guard adjusted 2.5 inches above the material being cut.  2) A Morgan 20" Chop Saw with the guard adjusted 6.5 inches above the material being cut.

29 CFR 1910.265(c)(4)(i): Walkway(s) were not of sufficient width to provide adequate passage and working areas:  JOB SITE: 11871 East State Highway E, Cadet, MO 63630, on or about 4/25/2016, in the production area, the employer failed to maintain a 28 inch minimum clearance in and around the chop and band saws, keep aisles and passageway clear without obstruction across or in the aisles.  The workers were exposed to numerous obstacles to safely exit out of the production areas, in case of an emergency.
