29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:   a) Job site, 235 North Duffy Road, Butler, PA: On or about April 26, 2016, during roofing activities, the employer did not ensure that all employees used fall protection systems/devices where exposed to fall hazards of at least 17 feet, 7 inches in height.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site, 235 North Duffy Road, Butler, PA: On or about April 26, 2016, during roofing activities, the employer did not ensure that a ladder used by employees to gain access to/egress from the roof was extended at least 3 feet above the roof line in that it was extended approximately 2 1/2 feet above the roof line.
