29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder: (a) Jobsite, 2626 Tunnel Boulevard, Pittsburgh, PA: On or about 04/20/16 a 32 ft. fiberglass ladder used to install metal brick ledge flashing was not set up at an angle so that the horizontal distance from the top support to the foot of the ladder was approximately 1/4 of the working length. The ladder was not extended and the top of the ladder was positioned against the wall at a height of approximately 14 feet above the ground and the feet were positioned on the sidewalk at a distance of approximately 7 feet from the wall and the horizontal distance was not maintained at 1/4 of the working length.
