29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe:  a) workplace, 400 Doremus Avenue, Newark, NJ  The employer did not ensure that rigging used was inspected prior to use.  A chain and hook rigging with a broken hook was used to hoist the excavator crusher attachment by hooking the usable hook back onto a chain link.    The violation occurred on or about 4-28-16

29 CFR 1926.600(a)(5): Equipment cab glass introduced visible distortion that affected the safe operation of the machine:   a) workplace, 400 Doremus Avenue, Newark, NJ  The employer did not ensure that cab glass on the Komatsu PC600  Hydraulic Excavator was maintained without visible distortion.  The front cab glass was broken with spider web type cracking covering the majority of the glass, obstructing and distorting the operator's view.     The violation occurred on or about 4-28-16
