29 CFR 1926.304(d): 29 CFR 1926.304(d): Each portable, power-driven circular saw withdrawn from the work did not automatically and instantly return the lower guard to the covering position of the saw: a) On or about 04/14/2016, the lower blade guard to a Ryobi 7 ¼ inch portable circular saw (serial no.: GSB123) was wired in the open position and thereby made inoperable. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) Back Yard On or about 04/14/2016, a vinyl siding mechanic was exposed to an 18 foot fall while working from the unguarded platform of a ladder jack scaffold which had not been provided with guardrails, nets or personal fall arrest equipment. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
