(a) Location - 1438 Main Street, Tewksbury, Massachusetts: On or about April 27, 2016, employees were using an electrical extension cord and a construction grade powertap in which the path to ground was not permanent and continuous; the ground pins were missing.

29 CFR 1926.405(g)(2)(iii): Flexible cords shall be used only in continuous lengths without splice or tap. Hard service flexible cords No. 12 or larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced. (a) Location - 1438 Main Street, Tewksbury, Massachusetts: On or about April 27, 2016, an electrical extension cord had a repair splice that did not maintain the outer sheath properties.
