29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about April 29, 2016, the employer failed to establish a written Hazardous Energy Control program to address the hazards of performing maintenance and repair work on equipment and machinery to include but not limited to: Manual Pipe Cutters. Maintenance is currently in the process of working on a the S5 S-Head machine which is locked out.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about April 29, 2016, the employer failed to provide adequate guarding on the Manual Cutting Machines (#30, 32 and 36), which are located in the main plant. Employees are cutting metal rods to various lengths and are exposed to a point of operation hazard due to their hands coming close to where the saw blade meets the metal rod that is being cut to size. Employees are placing rags into various parts of the machinery to soak up oil leaks and are exposed to rotating parts including but not limited to the rollers that the metal rods are guided on when cutting the rods. Employees are exposed to flying chips and sparks when cutting the metal rods when the saw blade strikes the metal rods that are being cut to size.
