29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: (a) On or about April 27, 2016, Durango Plastering & Drywall did not ensure that scaffolds and scaffold components were inspected for visible defects by a competent person before each work shift. A seven-tier scaffold in use by the demolition crew was not inspected for visible defects before each shift. Scaffold did not have properly installed guardrails, exposing employees to falls in excess of 50 feet. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.451(g)(1)(v): Each employee on a walkway located within a scaffold was not protected by a guardrail system (with a minimum 200 pound toprail capacity) installed within 9.5 inches (24.1 cm) of and along at least one side of the walkway: (a) On or about April 27, 2016, Durango Plastering & Drywall did not ensure that each employee on a walkway within a scaffold was protected by a guardrail system with a minimum 200 pound toprail capacity. A seven-tier scaffold in use by the demolition crew had spliced guardrails joined by a single strand of tie wire, exposing employees to falls in excess of 50 feet. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
