A seven-tier scaffold in use by the demolition crew was not inspected for visible defects before each shift. Scaffold did not have properly installed guardrails, exposing employees to falls in excess of 50 feet. A seven-tier scaffold in use by the demolition crew had spliced guardrails joined by a single strand of tie wire, exposing employees to falls in excess of 50 feet.
