29 CFR 1926.1408(a)(2): 29 CFR 1926.1408(a)(2): The employer did not determine if any part of the equipment, load line or load (including rigging and lifting accessories), if operated up to the equipment's maximum working radius in the work zone, could get closer than 20 feet to a power line:  a. On or about April 29, 2016, on the east side of the residential building, employees rigging, hooking wood roofing trusses and operating a crane were exposed to electrical shock when a hazard assessment was not performed.

29 CFR 1926.1408(d)(1): Part(s) of equipment, load line, or load (including rigging and lifting accessories) were allowed below an energized power line when the exceptions to paragraph (d)(2) did not apply:  a. On or about April 29, 2016, on the east side of the residential building, employees rigging, hooking wood roofing trusses and operating a crane were exposed to electrical shock when performing the work under energized power lines and less than 10 feet to the power lines.

29 CFR 1926.1408(g)(1): The employer did not train each operator and crew member assigned to work with the equipment on all of the following:   a.  On or about April 29, 2016, on the east side of the residential building, employees rigging and hooking wood roofing trusses on a crane below energized power lines were not trained regarding electric hazards.
