29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:     (a) Employees, who were working inside the trench excavation, with the electric saw and other equipment requiring electrical equipment power were exposed to electrocution, and electrical shock hazards from a 120 volt powered extension cord plugged into the jobsite electrical receptacle that had the power cord protective sleeve pulled out of the crimped clamp exposing the internal wiring. The power cord did not have a strain relief mechanism.
