29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): (a) On or about April 27, 2016, and at times prior, Durango Wood Company, Ltd. did not ensure that forklifts in need of repair, defective, or in any way unsafe were taken out of service until restored to safe operating condition. An Ingersoll-Rand forklift, model VR 90-B, was observed with a broken seat belt assembly, exposing operator to crush-by hazards. Employees were exposed to contact with electrical hazards as follows: 1- SCM Group Planar, model S630 X, was observed with signs of strain at the plug. 220V 2- Dewalt miter saw was observed with signs of strain where the cord enters the unit. 110V Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
