29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:  (a) West Scaffold: On or about April 28, 2016, and at times prior, employees were exposed to fall hazards from an elevated platform of approximately 9 feet 8 inches to the ground while trimming exterior windows.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  (a) West Scaffold: On or about April 28, 2016 and at times prior, employees were exposed to scaffold collapse and subsequent falls of approximately 9 feet 8 inches to the ground below while working from a scaffold setup on soft uneven ground.                          (b) East Scaffold: On or about April 28, 2016 and at times prior, employees were exposed to scaffold collapse and subsequent falls of approximately 9 feet 8 inches to the ground below while working from a scaffold setup on soft uneven ground.
