29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:      a)  On May 3, 2016, employees engaged in roofing activities were exposed to falls of greater than 25 feet.     Lindholm Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA inspection number 281360, citation 2, item 1, and was affirmed final order on April 25, 2012, with respect to a workplace located at 947 Leavitt St., Chicago, IL 60622.      Lindholm Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA inspection number 110142, citation 1, item 1, and was affirmed final order on January 17, 2012, with respect to a workplace located at 1225 Forest, Des Plaines, IL 60018.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigged support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:       a)  On or about May 3, 2016, an extension ladder used by employees to gain access to the working surface did not have its side rails extended at least 3 feet above the surface.     Lindholm Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1) which was contained in OSHA inspection number 712298, citation 2, item 1, and was affirmed final order on December 19, 2012, with respect to a workplace located at 17717 Oak Park Ave., Tinley Park, IL 60477.      Lindholm Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1) which was contained in OSHA inspection number 190481, citation 1, item 1, and was affirmed final order on March 15, 2012, with respect to a workplace located at 1100 Fair Oaks Ave., Oak Park, IL 60302.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

Lindholm Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 511458, citation 1, item 1, and was affirmed final order on August 14, 2102, with respect to a workplace located at 123 Melrose, Kenilworth, IL 60043.       Lindholm Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 190481, citation 2, item 1, and was affirmed final order on March 15, 2102, with respect to a workplace located at 1100 Fair Oaks Ave., Oak Park, IL 60302.
