29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):      a) Jobsite, 482 Galiffa Drive, Donora, PA 15033:  Observed on or about May 3, 2016, the employer did not establish an energy control program consisting of an energy control procedure and employee training to each employee required to perform any servicing or maintenance on a machine or equipment.  An employee was pinned between a rear tire and the frame of a Trojan 3000 material handler (serial number 2092470) while assisting in a recommended overhaul of the equipment.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings were not effectively closed:    a)  Main building, 482 Galiffa Drive, Donora, PA 15033:  Observed on or about May 6, 2016, unused openings on an electrical panel housing a switch labeled 'Paddle Mixing Controller' were not effectively closed.  The openings could allow for the accumulation of dust and debris inside the electrical cabinet which could create a fire hazard.
