29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Rainbow Falls waterslides; On or about May 4, 2016 and at times prior thereto, employees were exposed to fall hazards of up to 28 feet to the ground below in that employees were required to wax the slides without being provided personal protective equipment by the employer.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: a) Water park; On or about May 4, 2016 and at times prior thereto, employees who wax and paint at the facility were exposed to potentially hazardous chemicals in that the employer failed to maintain a written hazard communication program as required by the standard.
