Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that caused or were likely to cause death or serious physical harm in that employees were exposed to a fall hazard.  White Oak Operating, LLC, Rig #5, Noble #3, Tom Rd., Gueydan, LA:  On or about April 21, 2016, employee(s) engaged in drilling operations were exposed to a fall hazard when the chain and hook holding the rig floor plate became disengaged causing an employee to fall 17 feet.
