Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught-in hazards while using a dumbwaiter (food elevator):    (a) On or about May 3, 2016, employees operated a dumbwaiter (food elevator) system that was not equipped with safety devices in that at the lower floor the hoistway landing doors were kept open while the dumbwaiter (food elevator) was in use.  In addition at the upper and lower floors, the hoistway landing doors did not guard the full height of the hoistway openings and the hoistway doors were not equipped with interlocks to prevent the dumbwaiter from being operated while the hoistway doors were opened.  Employees were exposed to being caught in the dumbwaiter.
