Employees exposed to falls of ten feet. b) On or about 5/4/2016, Aggregate building: An inclined walkway leading to the silo area was lacking a midrail and top rail on one side. Employees exposed to falls of up to eighteen feet. c) On or about 5/4/2016, Lower cat house area: An access platform to the lower cat house was lacking a guardrail system. Employees exposed to falls of ten feet. d) On or about 5/4/2016, Aggregate building: Platforms at the top of the aggregate bins were not provided with guardrail systems. Employees were exposed to falls of up to seventeen feet. Abatement documentation is required for this item.
