29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).      On or about May 5, 2016, at a job-site located at 2155 Championship Cir SE, Massillon, Ohio 44646, the employer did not ensure adequate cave-in protection was used to protect an employee working inside a trench with near vertical side walls and a depth of approximately 7 feet.  The trench was determined to have type "C" soil
