On or about May 9, 2016 during the installation of roofing material at the job site located next to 11817 Walton Cir NW, Uniontown, Ohio 44685; the employer did not ensure employees working on the roof of a new construction home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.
