29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  On or about May 10, 2016, Christain O. Helmuth had employees exposed to fall hazards of up to 45 ft., while operating an aerial lift, and the employer did not provide the required scaffold training, which would include use of aerial lifts.  This training would cover:    a. The nature of any electrical hazards, fall hazards, and fall object hazards in the work area,  b. The correct procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems being used;  c. The proper use of the scaffold, and the proper handling of materials on the scaffold;  d. The maximum intended load and the load carrying capacities of the scaffolds used; and  e. Any other pertinent requirements of this subpart, Subpart L  Scaffolds.

The employer did not provide construction safety training to employees, in order to provide them with the knowledge necessary to recognize the hazards of falling and the safety requirements associated with fall protection, as well as the proper work procedures to follow, in order to minimize and eliminate the fall hazards.
