29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:  a) On or about 5/11/2016, on the fly ash silo, the employer allowed employees to access a ladder platform which was not offset or provided with a swinging gate.   b) On or about 5/11/2016, on the cement silo structure, the employer allowed employees to access a first level (above the ground) platform in order to conduct equipment inspections and preventative maintenance and did not ensure that the ladderway floor opening was guarded by a swinging gate or equivalent.   c) On or about 5/11/2016, on the cement silo structure, the employer allowed employees to access a second level (above the ground) platform in order to conduct equipment inspections and preventative maintenance and did not ensure that the ladderway floor opening was guarded by a swinging gate or equivalent.
