29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Roof of enclosed front porch:  An employee was engaged in painting operations on the front of the house while standing on a roof approximately ten (10) feet above the concrete below without the use of fall protection.  (b) (a) Roof of enclosed front porch:  An employee was engaged in painting operations while standing on an aluminum stepladder placed on a roof approximately ten (10) feet above the concrete below without the use of fall protection.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.  Front of house:  An employee was engaged in painting activities from an aluminum extension ladder that was placed on the tread of a step that was covered with a tarp in front of the door to the house.  The top of the ladder was resting on the gutters and the ladder was not secured to prevent accidental displacement exposing the employee to a fall of approximately seven (7) feet to the concrete steps and ground below.
