29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) At 102 N Missouri Street in Potosi, MO 63664 - On or about 05/11/2016, the employer failed to ensure that each employee working on a steep-roof (5-in-12 pitch or greater) with unprotected sides and edges approximately 40-45 feet above a solid concrete surface was protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) At 102 N Missouri Street in Potosi, MO 63664 - On or about 05/11/2016, employees were exposed to elevated fall hazards of approximately 40 feet to a solid concrete surface where a portable ladder was used to access an upper landing surface and the ladder's side rails only extended 1 and 1/2 feet above the upper landing surface being accessed.
