29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.    a. On or before May 12, 2016 where employees were removing the roof of a building with a chain saw, eye and face protection was not provided, thereby exposing the employee to struck-by hazards.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     a. On or before May 12, 2016 where employees were removing the roof of a building, employees were not protected from falls, thereby being exposed to a 30 foot fall hazard.

29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey shall be made, by a competent person, of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure. Any adjacent structure where employees may be exposed shall also be similarly checked. The employer shall have in writing evidence that such a survey has been performed.     a. On or before May 12, 2016 where employees were removing the roof of a building with the sides and interior already partially removed, a demolition survey was not conducted, exposing employees to 30 foot fall and other hazards.
