The employer did not maintain a record of injuries and illnesses that was equivalent to the OSHA Form 300. b) On or about May 12, 2016 the employer did not keep a 2013, 2014, 2015 Summary of Work-Related Injuries and Illnesses that included the following information: a certification signature by a company executive acknowledging their review of the information; the total number of injury and illness cases according to there severity; the total number of days employees were confined to restricted or lost work days; the total number of injury and illness cased according to their type. The employer did not maintain a summary record of injuries and illnesses that was equivalent to the OSHA Form 300-A
