29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.    On or about May 13, employees were working on the roof of a residence and were not provided with any fall protection.  Employees were exposed to a fall of at least 6 feet to the ground below.

29 CFR 1926.503(c)(3): 29 CFR 1926.503(c)(3): The employer did not ensure employees were retrained when inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill.    On or about May 13, 2016, the employer did not adequately instruct his employees in the recognition and avoidance of unsafe conditions on the work site.
