29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4):  (a) Location - 126 Merrimack Road, Methuen, MA:  On or about 5/11/2016, a Makita angle grinder was used with a Type 27 abrasive wheel to perform grinding on pipe with no safety guard.

29 CFR 1910.243(c)(5)(i): All wheels were not closely inspected and sounded by the user to ensure they have not been damaged in transit, storage, or otherwise immediately prior to mounting. The spindle speed of the machine was not checked before mounting of the wheel to ensure that it did not exceed the maximum operating speed marked on the wheel:  (a) Location - 126 Merrimack Road, Methuen, MA:  On or about 5/11/2016, the Type 27 abrasive wheel mounted on the Makita angle grinder was not appropriately rated for the operating speed of the tool.
