29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to: Two instances: a) On or about May 10, 2016, at the above addressed jobsite, the employer did not provide specific energy control procedures as required by the Standard to employees who had been operating and maintaining the Model 1300 Tub Grinder which had pneumatic, hydraulic, mechanical gravity and electrical energy sources, thus exposing employees to electrical shock, crushing, caught-in between and laceration hazards from pressurized hydraulic fluid air. b) On or about May 10, 2016, at the above addressed jobsite, the employer did not provide specific energy control procedures as required by the Standard to employees who had been operating and servicing vehicles which had pneumatic, hydraulic, mechanical gravity and electrical energy sources, thus exposing employees to crushing, caught-in between and laceration hazards from pressurized hydraulic fluid air.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: Two instances: a) On or about May 10, 2016, at the above addressed jobsite, employees who had been operating and maintaining the Model 1300 Tub Grinder which had pneumatic, mechanical gravity and electrical energy sources, had not been trained in the specific energy control procedures that were provided by manufacturer, thus exposing employees to crushing, caught-in between and laceration hazards from pressurized hydraulic fluid air. b) On or about May 10, 2016, at the above addressed jobsite, the employer did not provide adequate training in specific energy control procedures as required by the Standard to employees who had been operating and servicing vehicles which had pneumatic, hydraulic, mechanical gravity and electrical energy sources, thus exposing employees to crushing, caught-in between and laceration hazards from pressurized hydraulic fluid air.
