29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    O.K. Exchange Ltd. dba O.K. Industries, Inc. - Fostoria, Ohio: On or about May 9, 2016, employees working elevated platform located at the Line 3 grinder were exposed to a 7.5-foot fall hazard.  The employer did not ensure guardrails were in place on all sides of the platform to protect against falls to the ground below.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.     O.K. Exchange Ltd. dba O.K. Industries, Inc. - On May 9, 2016, an employee sustained fracture injuries due to a fall from an elevated platform.  The event resulted in an in-patient hospitalization which was not reported within 24 hours.
