29 CFR 1910.23(a)(9): Floor hole(s) into which persons could not accidentally walk were not protected by securely held covers:  1st Floor Lunch Room: On or about May 26, 2016, and times prior thereto, the employer did not protect the floor holes openings greater than one inch wide with covers. This condition exposes employees to trip and fall hazards.
