29 CFR 1926.1126(d)(2)(i): The employer using the scheduled monitoring option did not perform initial monitoring to determine the 8-hour time-weighted average exposure to chromium (VI) for each employee on the basis of a sufficient number of personal breathing zone air samples to accurately characterize full shift exposure on each shift, for each job classification, in each work area:  (a)  In the work area identified as CA20 - 22A/B, on or about March 18, 2016 and at times prior to, an employee was performing carbon arc gouging on stainless steel which has the potential to generate airborne concentrations of chromium (VI).  The employer failed to perform initial employee exposure monitoring for this task exposing employees to respiratory hazards.
