29 CFR 1910.212(a)(1):   One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a/  Inside of the Maintenance Shop, the rotating parts belonging to the Turn-Pro Milling & Drilling Machine, m/n 126-2655, s/n 43L1313, were not guarded.

29 CFR 1910.215(a)(4):   Work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest was not securely clamped after each adjustment:    a/  Inside of the Main Plant, the work rest belonging to the Dewalt 8" Heavy-Duty Bench Grinder, m/n DW758, s/n 2008-08-YL1853, was approximately .5 inches from the abrasive wheel.  The work rest was not secured into place.

29 CFR 1910.305(b)(1)(i):  Openings through which conductors enter were not effectively closed:    a/  Inside of the Main Plant, specifically near the North Splitter, live conductors entered through open conduit that was approximately two inches in diameter.  The conduit was not effectively closed.
