29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers:      (a) Whipple & Brewster Corporation at 601 Hallam St., Aspen, CO 81611:  On and before May 10, 2016, the employer did not ensure that employees were protected from stepping into or through holes by covers, in that the cover to the elevator floor hole near the upper landing area was removed.  The floor hole measured approximately 3.5 feet in diameter.  An employee sustained injuries after he stepped into the uncovered hole and fell to the lower level.  This condition exposed employees to a fall hazard of approximately 12 feet.

29 CFR 1926.502(i)(4):  All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard:     (a)         Whipple & Brewster Corporation at 601 Hallam St., Aspen, CO 81611:  On and before May 10, 2016, the employer did not ensure that employees were adequately warned about floor holes by marking the covers with the words "HOLE" or "COVER". The floor hole cover on the first floor was marked with the words "Stay Off" which did not adequately inform employees of the hazard located underneath it.

29 CFR 1926.304(h)(1):  Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut:       (a)         Whipple & Brewster Corporation at 601 Hallam St., Aspen, CO 81611:  On and before May 10, 2016, the employer did not ensure that a circular hand-fed crosscut table saw was guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut. This condition exposed the employees to amputation hazards.
