29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:                     a) On or about May 1, 2016, in the reel room of the facility, employees were required to step on or kick a floor switch in order to release and close gaps between paper rolls that weighed approximately 1200 pounds each. The floor switch was located between the paper rolls. Employees were thereby exposed to struck-by hazards associated with rolling 1200 pound paper rolls. The employer did not develop and utilize specific procedures for controlling hazardous energy as required by 29 CFR 1910.147.
