29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Instances including, but not limited to, the following: a) Job site, 11 Talbot Avenue, Rankin, PA: On or about May 18, 2016, while operating the Grizzly mill, serial number 23395, located in the maintenance shop, the employer did not ensure that machine guarding was provided to prevent contact with the rotating tool bit, exposing the employee to amputation type hazards. b) Job site, 11 Talbot Avenue, Rankin, PA: On or about May 4, 2016, while operating the Grizzly lathe, serial number 6478, located in the maintenance shop, the employer did not ensure that machine guarding was provided to prevent contact with the rotating tool chuck, which had chuck jaw projections determined to be of at least 1 inch or more, exposing the employee to caught on/in type hazards. c) Job site, 11 Talbot Avenue, Rankin, PA: On or about May 17, 2016, while operating the MSC Industrial band saw, serial number 0336300, located in the maintenance shop, the employer did not ensure that machine guarding was provided to prevent contact with the unused portion of the saw blade, exposing the employee to amputation type hazards. d) Job site, 11 Talbot Avenue, Rankin, PA: On or about May 17, 2016, while operating the Racine Repricocating Hacksaw unit, located in the maintenance shop, the employer did not ensure that machine guarding was provided to prevent contact with the unused portion of the saw blade, the hydraulic mechanism controlling saw's upward/downward movement or the reciprocating arm on the front of the unit, exposing employees to crushing and amputation type hazards. e) Job site, 11 Talbot Avenue, Rankin, PA: On or about May 17, 2016, while operating the DoAll band saw unit, located outside the maintenance shop, the employer did not ensure that machine guarding was provided to prevent contact with the unused portion of the saw blade, exposing employees to amputation type hazards. f) Job site, 11 Talbot Avenue, Rankin, PA: On or about June 30, 2016, while operating the welder 9 unit, the employer did not ensure that appropriate machine guarding was provided over the welding area, as employees were reaching into the machine to remove pliers from the steel parts being welded together, exposing the employees to crushing type hazards. g) Job site, 11 Talbot Avenue, Rankin, PA: On or about June 30, 2016, while working to perform truck loading operations at mill 6, the employer did not ensure that machine guarding was provided over the pusher unit at the end of the press line, exposing the employee to crushing type hazards. h) Job site, 11 Talbot Avenue, Rankin, PA: On or about June 30, 2016, while working to perform truck loading operations at mill 6, the employer did not ensure that machine guarding was provided over the swing tables at the end of the press line, exposing the employee to caught on type hazards.
