29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    On or about May 19th, 2016 at the above addressed job site, employee who perform cleaning of screens was exposed to chemical burns from corrosive chemicals that contain Periodic Acid, due to not being provided suitable facilities for quick drenching or flushing of the eyes and body.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    On or about May 19th, 2016 at the above addressed job site, employees pulling merchandise out of racks were exposed to a fall hazard of approximately 12 feet due the use of an attachment on a Toyota Forklift Truck (Model FGCU25) without the manufacturer's prior written approval.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  On or about May19th, 2016 at the above addressed jobsite, the employer did not develop or implement a written hazard communication program to address hazards related to substances that contain chemicals such as but not limited to Periodic Acid, Acetone, Propane and Calcium Carbonate for employees who perform silk-screening operations.
