29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system:   (a). Worksite located at Bridge #248, Pleasant Valley, Missouri- Employees were observed working and walking near the edge of the bridge and near openings on the deck and were not protected from falling by the use of fall protection equipment.

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:   (a). Worksite located at Bridge #248, Pleasant Valley, Missouri- Employees were not protected from falling when working near the edges and near floor openings while hoisting materials in to place on the deck.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a). Worksite located at Bridge #248, Pleasant Valley, Missouri- Employees were exposed to a fall hazard in that the ladder being used to access the bridge deck was not extended at least 3 feet above the upper landing surface. The ladder was also placed in poor location near the edge of a retaining wall which had a fall distance of more than 6'.
