OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by/caught-in between hazards associated with operating elevated machinery in reverse along pathways with overhead obstructions:     On or about May 20, 2016, on SRU Road (unpaved gravel road) inside the Calumet Shreveport Lubricants and Waxes refinery, the employer failed to ensure that employees operating/driving aerial lifts (JLG, Model 860SJ) in reverse or "backing" with the jib boom elevated were protected from contact with overhead piping systems.
