29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  (a)  The Crown SC 4000 Series electric forklift (Serial Number 9A122432) had forks attached, however, the nameplate stated there was a Paper Roll Clamp R25F-RCF-06A attachment.  The nameplate did not accurately show the approximate weight of the truck and the capacity of the forklift.
