29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation:    East entrance, middle bay:  On or about May 25, 2016, the employer allowed employees to operate a 5 ton crane (#109) with a damaged cable.  The crane is used to lift heavy objects in the shop.
