29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around each hole:  a) 44 Marjorie Street, Staten Island, NY 10309. The employer allowed his employee to work on a walking/working surface in close proximity to an unprotected stairwell aka hole exposing the employee to fall hazards.   Incident occurred on or about 04/08/2016.

29 CFR 1904.39(a)(2): An oral or written report of an employment accident resulting in an employee being admitted to the hospital or the loss of an eye or an amputation was not made to the nearest Area Office of the Occupational Safety and Health Administration (OSHA) within 24 hours after the occurrence:   a) 44 Marjorie Street, Staten Island, NY 10306. Notification of a work related incident that occurred on or about 04/08/2016 which resulted in an in-patient hospitalization was not made by the employer to OSHA within 24 hours after that occurrence.   Incident occurred on or about 04/08/2016.
