29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:    a)    At the 7002 Hodgson Memorial Dr. jobsite: On or about May 25, 2016, and at times prior thereto, employees were exposed to struck-by hazards, in that the lower extremities of a ground worker were not adequately protected as metal was cut and sections were raised up a portable ladder to facilitate roofing activities.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system:    a)    At the 7002 Hodgson Memorial Dr. jobsite: On or about May 25, 2016, and at times prior thereto, employees were exposed to fall hazards of at least 19.5 ft., in that fall protection was not worn while engaged in roofing activities on a 4:12 low-slope commercial roof.

29 CFR 1926.1053(b)(21): Each employee did not use at least one hand to grasp the ladder when progressing up and/or down the ladder:    a)    At the 7002 Hodgson Memorial Dr. jobsite: On or about May 25, 2016, and at times prior thereto, the employer failed to ensure that workers used at least one hand to grasp the ladder, as they used a 28 ft. portable ladder to facilitate roofing activities.
