29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about May 24, 2016, at a worksite located at 2108 Lander Road, Mayfield Heights , OH 44124, during construction activities including, but not limited to commercial roofing activities, employees were exposed to fall hazards of approximately 10.5 to 11 feet. The employer was not enforcing the fall protection system/device during the roofing activities.
