29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a)  40 Allen Street Job Site - On or about 5-25-16 - Employee was exposed to a fall hazard of ten feet, six inches while placing pieces of lumber on open floor joists to support a load of lumber being placed on the second floor of the building with a fork lift.  No fall protection systems were being used.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a)  40 Allen Street Job Site - On or about 5-25-16 - Employee was exposed to a ten foot, six inch fall hazard by using a portable aluminum extension ladder that did not extend three feet above the second floor work area.
