29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    a) On or about 05/27/16 in the shop area; employees were exposed to caught between hazards of the moving heads or being struck by moving equipment, when they did not attach locks to the energy source(s) before performing service and maintenance work, such as but not limited to, changing over the puller heads, wheel replacement, greasing/lubricating equipment inside the back cabinet of the stirrup benders.       b) On or about 05/27/16 in the shop area; employees were exposed to caught on hazards of the rotating spindle, when they did not attach lock(s) to the energy source(s) before performing the changing of the head on the table top bender.      c) On or about 05/27/16 in the shop area; employees were exposed to caught between hazards of the moving rollers, when they did not attach lock(s) to the energy source(s) before performing the lubrication of the discharge rollers on the shears. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about 05/27/16 in the production area; employees were exposed to the amputation hazard of the unguarded blade when the safety interlock for the inlet guard on Shear #1 was missing the roller arm preventing it from operating.      b) On or about 05/27/16 in the production area; employees were exposed to the amputation hazard of the unguarded blade when the safety interlock for the inlet guard on Shear #2 was wired open to prevent it from operating.     c) On or about 05/27/16 in the production area; employees were exposed to the amputation hazard of the unguarded blade when the inlet guard was open on the top allowing access to the shear blade on Shear #2.      d) On or about 05/27/16 in the production area; employees were exposed to the crushing hazard of the moving puller head when the safety interlock on the cover of the circle bender was defective allowing the cover to be opened while the machine continued to operate.
