(a) At the jobsite, during roof tear-off and repair: On or about 5/27/16, employees did not use protective eyeware (safety glasses) for protection from flying debris.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) At the jobsite, during roof shingle rip-off and replacement: On or about 5/27/16, employees were exposed to a fall hazard of approximately 9 to 15 feet when working on the main roof of the house; pitch of the roof was reportedly 5:12 or 6:12. Although roof jacks with scaffold planks were installed at exposed eave edges, employees did not use conventional fall protection equipment.
