29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  (a) At the jobsite, during roof tear-off and repair:  On or about 5/27/16, employees did not use protective eyeware (safety glasses) for protection from flying debris.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   (a) At the jobsite, during roof shingle rip-off and replacement:  On or about 5/27/16, employees were exposed to a fall hazard of approximately 9 to 15 feet when working on the main roof of the house; pitch of the roof was reportedly 5:12 or 6:12.   Although roof jacks with scaffold planks were installed at exposed eave edges, employees did not use conventional fall protection equipment.
