29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    Jobsite: 8 Beresford Road Brookline, MA : Workers were not informed of the hazard of operating gasoline powered equipment and the potential of carbon monoxide exposure and poisoning. Workers were not instructed to use equipment such as ventilation blowers to ensure carbon monoxide levels reduced in the work space.

29 CFR 1926.55(a): Employee(s) were exposed to material(s) at concentrations above those specified in the Threshold Limit Values of Airborne Contaminants for 1970 of the American Conference of Governmental Industrial Hygienists:     Basement, 8 Beresford Road Brookline, MA:  On 5-26-16 a worker was exposed to 207 parts per million(ppm) of carbon monoxide, calculated as an 8 hour time weighted average, in excess of the Threshold Limit Value of 50 ppm, while operating a gasoline powered concrete trowel.

29 CFR 1926.55(b): Feasible administrative or engineering controls were not implemented to reduce employee exposures:  Basement, 8 Beresford Road, Brookline, MA: On 5-26-16 administrative and /or engineering controls were not implemented to reduce airborne concentrations of carbon monoxide below the threshold value of 50 ppm, while a worker was operating a gasoline powered concrete trowel.   Controls may include but are not limited to:  1. Use of local ventilation to exhaust and/or push in fresh air into the confined basement.   2. Use of a carbon monoxide detector in the work space to ensure workers are warned when levels exceed 50 ppm.  3. Substitute gasoline powered equipment with electrically powered equipment.
