29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury.  On or about 26 May 2016 and at times prior thereto, the employer failed to provide hazard recognition and safety training to four employees performing framing activities on a residence under construction, exposing the employees to hazards including, but not limited to, being struck in the eyes and head by flying or falling objects and falling from a height of approximately 10 feet, 1 inch to the ground below.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 26 May 2016 and at times prior thereto, an employee without head protection was cutting and throwing lumber from the ground to another employee working above, exposing the employee to the hazard of being struck in the head by falling materials.

On or about 26 May 2016 and at times prior thereto, two employees without eye protection were performing framing activities using pneumatic nail guns and hammers, exposing the employees to the hazard of being struck in the eyes by flying objects.

On or about 26 May 2016 and at times prior thereto, three employees were performing framing activities on a residence under construction without fall protection, exposing the employees to the hazard of falling approximately 10 feet, 1 inch to the ground below.
