29 CFR 1926.416(a)(1): Employee(s) were permitted to work in proximity to electric power circuits that were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) 100 South West Street Carlisle, PA - The employer did not protect the existing 7200 high voltage electrical power lines from employees that were working in close proximity to electric power, thereby exposing employees to injuries, on or about May 26, 2016. No abatement certification or documentation required.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: a) 100 South West Street Carlisle, PA - Employees were exposed to an energized electrical hazard while performing metal rain gutter work on a residential house. The employer failed to instruct each employee in the recognition and avoidance of 7200 volt residential power approximately 8 feet 9 inches away from where the employees were working, thereby exposing employees to an electrical shock, on or about June 1, 2016. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
